This is Varilight part no. KQP401W.

The V-Com dimmer series, provides leading-edge (triac) control for dimmable LEDs. For trailing edge dimming control, choose a V-Pro dimmer instead.

Each dimmer module will control up to 10 dimmable LED bulbs with a total wattage up to 100w.
